Terms & Approval
By filling out this form you and your restaurant agree to participate in StreetSmart Canada’s initiative to raise funds for the food banks. The terms of the agreement are as follows:
- You agree to place the StreetSmart Canada card on all your restaurant tables for the duration of the 2018 campaign.
- You agree to have your servers invite guests to donate to StreetSmart.
- You agree to add StreetSmart Canada as a line item to your billing system in order to facilitate the collection of donations (StreetSmart Canada collections will not be a taxable item on the total table amount and the donation should appear separately on the bill).
- You agree to forward all donations collected to StreetSmart Canada by January 31st, 2018.
- You agree to allow the use of your name in the media campaign to raise awareness for the StreetSmart Canada Project initiative.
